Access to RDNS Services
Making a referral to RDNS is as easy as either;
- completing the secure online RDNS referral form
- printing and faxing the RDNS referral form to us on 8378 5383
- Calling us on 1300 364 264
Please ensure that referrals for medication management are accompanied by a Medication Authority.
Referrals need to be received by 3pm (Mon-Fri) to enable a nursing visit the following weekday.
